I love the Lord Jesus Christ with all of my heart and I am committed to living wholeheartedly for Him.  I am also conscience-bound to follow the Bible alone and as a result I must ask you to please remove me from membership in the Seventh-day Adventist Church.  Please do not send anyone to visit me as I have made my decision and I am not comfortable with a visit.  I do request that you send me written notification that you have acted promptly upon my request.  I would like to thank you in advance for giving this matter your attention.
